If the member dies while off-duty, the burial benefit and supplementary benefit for burial costs are paid to his/her dependent(s). If the member’s dependent dies, the family burial benefit and supplementary benefit for family burial costs are paid to the member.

If a member without dependents dies, the cost directly incurred by the burial will be paid to the individual who actually took care of the burial. The amount is not to exceed to the total amount of the burial benefit and supplementary benefit for burial costs.

Submit the claim form for the burial benefit, family burial benefit, or the supplementary benefit for burial costs, along with a copy of the burial or cremation permit issued by the mayor of your local city or town, or the police chief. If someone other than a dependent claims the burial benefit, attach a receipt for the payment directly incurred by the burial.
